The Captain's log on the USS Excelsior was the method used by the commanding officer to officially record the ship's missions.

Captain Hikaru Sulu made such an entry at the conclusion of his mission in the Beta Quadrant in 2293. (Star Trek VI: The Undiscovered Country)

In 2373, Captain Janeway studied the Excelsior logs to learn more about the Excelsior's journey into the Azure Nebula. According to Janeway, "It would seem that Captain Sulu decided not to enter that journey into his official log. The day's entry makes some cryptic remark about the ship being damaged in a gaseous anomaly and needing repairs, but nothing else." (VOY: "Flashback")

One inaccuracy with Janeway's remark and Sulu's entry was that the Excelsior's encounter with the "gaseous anomaly", was actually with a subspace shock wave, and his later encounter with the nebula actually occurred two months later.
