Yeoman Doris Atkins was a 23rd century Starfleet officer who served aboard the USS Enterprise under the command of Captain James T. Kirk.
In 2267, Atkins handed a report to Captain Kirk for him to sign. Kirk, who was afflicted at the time with a disease that caused rapid aging, signed the report and returned it to Atkins. Moments later, he demanded that Atkins let him read and sign the report, having forgotten due to his deteriorating condition that he had already signed it. She later reluctantly attested to Kirk's forgetfulness during the hearing convened to determine his competency for command.(TOS: "The Deadly Years")
