Elba II was the second planet in the Elba system. The planet was affiliated with the United Federation of Planets.

The planet had an atmosphere that was poisonous to humanoids.

Prior to stardate 5718, the Federation took advantage of the inhospitable environment for the establishment of an asylum for the criminally insane – the Elba II asylum, The asylum was administered by a governor. In 2269, the governor was Doctor Donald Cory.

In that year, the Federation starship USS Enterprise delivered a revolutionary new medicine to this asylum. The hope with this medicine was that it would permanently eliminate mental illness. A patient at this asylum, Captain Garth of Izar, a former Starfleet Fleet Captain, was treated with this medicine. (TOS: "Whom Gods Destroy")

According to the Star Trek Concordance, "the planet is named after the island of Napoléon I's first exile."
