Leonard H. McCoy was the chief medical officer of the Terran Empire's ISS Enterprise. His sickbay was filled with torture devices that appalled Leonard McCoy of the prime universe when the two accidentally swapped places in 2267.

So alike were the actions of the two McCoys that they both spilled acid on tables in their respective sickbays, creating a duplicate burn mark on each. (TOS: "Mirror, Mirror")

According to the Mirror Universe novel The Sorrows of Empire, this McCoy tortured his father David McCoy to death on the orders of the Terran Empire. He died of xenopolycythemia in 2269, as the ISS Enterprise either never encountered or destroyed the Fabrini ship on which his prime counterpart discovered a cure for the disease. He was succeeded as chief medical officer by the mirror universe counterpart of Dr. M'Benga, who continued to serve in that position until at least 2287.
