A subspace anomaly was a very broad term for any sort of extraordinary disruption in subspace. Often, they could take the form of irregularities such as distortions, fractures, tears, or ruptures. Oftentimes they could damage technology and personnel, altering physics and the Human brain.

A subspace anomaly was one of several causes proposed to explain the disappearance of the Ravinok in 2366. (DS9: "Indiscretion")

In 2371, the USS Voyager was struck by a kinoplasmic radiation surge from a subspace anomaly. This caused The Doctor's program to malfunction, subjecting him to a delusion in which he was Human and Voyager was a hologram. (VOY: "Projections")
