Tarakis is a planet in the Delta Quadrant, that was evacuated sometime in the early 21st century. (VOY: "Memorial")

Before that, the planet was inhabited with Nakan colonists. They were ordered to evacuate, but due to an accident, they were all murdered by the militant group who were to evacutate them. The group was led by Commander Saavdra.

The memorial

A memorial for the Nakan colonists was erected on Tarakis. It contained a synaptic transmitter that forced anyone who came near it to relive the massacre.

In 2376, four members of the USS Voyager crew encountered the memorial while on an away mission in the Delta Flyer. When they returned to Voyager, they began to have hallucinations about being in combat. After retracing the path of the mission, most of the crew became affected. An investigation revealed that the hallucinations were the memories of soldiers involved in the slaughter, uncovering the existence and function of the memorial.

Having been abandoned for 300 years, the memorial was beginning to deteriorate. Before leaving, Captain Kathryn Janeway ordered the memorial repaired so that no one would forget what happened there. Janeway also ordered that a warning buoy be placed in orbit of Tarakis, so that passing ships would know what to expect.
