Dr. Theodore Haskins was a male Human civilian in the 23rd century. He was a scientist for the North American Continent Institute. In 2236, he was a member in the SS Columbia expedition.
That same year, he was killed when the survey ship crashed on Talos IV in 2236. The Talosians recreated an image of Haskins in 2254 as part of their attempt to lure the crew of the USS Enterprise to the planet's surface. (TOS: "The Cage", "The Menagerie, Part I")

Haskins was played by Jon Lormer.
Haskins was named for associate producer Byron Haskin. (Star Trek Encyclopedia 3rd ed., p. 184) In the second revised script of "The Cage", the character was consistently referred to as "1st Survivor", even after he announced his name.
Apocrypha
Haskins appeared in the Star Trek: The Original Series novel Burning Dreams where, by the late-2220s, he had a full professorship in geology, teaching at a prominent university on Earth. The novel further explained that he saved the life of his fiancé, Vina, by placing her in a refrigeration unit prior to the SS Columbia's crash on Talos IV in 2236.
